Candidate must attend training in person during this time. Remote-hybrid opportunity after successful onboarding and orientation.Our Intake and Referral department is the heart of Home Health & Hospice operations, and we want you on our team! We have an open position for a registered nurse who is interested in supporting an exceptional patient experience, every time. Our registered nurses and team assistants work together to make certain we have all required information and documentation for a complete referral, and coordinate with referring providers and internal directors and managers to support patient access to our services. This position offers an opportunity for a registered nurse to use their clinical experience to evaluate patient eligibility for our services and support patients, their families, community members and referring providers in understanding the breadth and depth of the services we offer.Schedule is every other Fri/Sat/Sun, and then 3 week days.Position-Specific Minimum Requirements
- 3 years worked experience as a Registered Nurse
- Live within driving distance to the Colchester, VT office or willingness to relocate
- Prior home health experience preferred
Requirements:
- Current RN licensure recognized by the State of Vermont required.
- Appropriate experience in specific clinical area. Varies by unit.
