Job Description

ServiceNet is seeking a Home Study Coordinator to join our Family Based Living team. This role will conduct comprehensive home study assessments for individuals and families seeking to provide care for adults in Family Based Living placements. Utilize clinical assessment skills to determine eligibility, ensure regulatory compliance, and facilitate placement processes in collaboration with program leadership.Key Responsibilities

  • Stay current with DDS and MassHealth regulatory requirements
  • Respond to planned and emergency placement needs
  • Conduct in-home safety assessments and vetting of care providers
  • Complete detailed home study narratives and clinical assessments
  • Screen, approve, and maintain respite provider records
  • Oversee background checks (CORI, SORI, fingerprinting, and driving records)
  • Manage intake, home studies, and licensing compliance
  • Represent Family Based Living with funding agencies at regional/local levels
  • Additional duties as assigned my management

Qualifications

  • Education: Bachelor’s in Social Work, Psychology, or related field (Master’s preferred)
  • Experience: 5-7 years in a related professional role
  • Licensure: LSWA or LSW preferred
  • Skills: Strong written, verbal, and data management skills; multilingual a plus
  • Other: Valid driver’s license, reliable transportation, ability to travel

Work Conditions & Physical Demands

  • Field and office-based work; travel required
  • Frequent sitting, standing, reaching, and stair climbing
  • Rarely lifting/transferring person with or without assistance of co-worker with or without mechanical lift
  • Occasional lifting (up to 25 lbs)
  • Ability to work with confidential data and manage unexpected disruptions

I grew up on a farm, showed dairy cows when I was in high school, studied Animal Science at Smith Voc, and worked on several farms raising llamas, goats and alpacas before I came to Prospect Meadow. And I’ve since learned at least as much from the farm hands as I have brought to them. They are an incredible group of caring, curious, nurturing people who always want to know how I am, how my dog is; and they are eager to learn what I share with them about farming. They’ve taught me how to communicate very specifically, to include important details, and to check to make sure I’ve been understood-not just to assume. And they’ve taught me how to slow down, take a breath, think.
